Most major insurance carriers preclude us from billing two different types of insurances on the same day. If this is true for your insurance(s), and you have a medical condition or diagnosis necessitating a medical exam, there are two alternatives.

1. We can schedule your medical exam and vision exam on separate days to utilize both insurances.
2. We can perform a combined medical and vision exam all together in one visit. This will be billed to your medical insurance, and you will be responsible to pay the medical copayment + refraction (and/or contact lens) fee.

Please bring all current insurance card(s), photo identification, and referral information (if required by your insurance). Please also take a look at our patient forms that will be provided in the office; in general, bringing the completed forms to your visit may help expedite your appointment.
